Our goal is to make you feel as comfortable as possible. Please arrive a few minutes early to allow time to complete our registration forms if you have not previously done so. Our team will work with you to develop a treatment plan that helps you achieve and maintain a healthy smile.
When you come in for your initial visit, please bring your dental insurance card and arrive early to complete your initial paperwork, if you haven't already done so.
Reminders for your first visit:
- If you are unable to complete our forms prior to your visit, please arrive 10-15 minutes early so you can complete them in our office.
- Bring your dental insurance card and ID
- Bring any prior dental records that are available to you
- Patients under the age of 18 should bring their parent or legal guardian to the appointment
- Compile a list of your current medications
- If you need antibiotics before dental work, please take them as prescribed. If you are unsure whether you need antibiotics, please consult your doctor before your visit.
